Our Young Persons Care Coordinator works as part of the Wellbeing Team at The Blandford Group Practice. The role as a ‘Young Persons Care Coordinator’ is to support young people aged between 5 and 25 who are patients of the practice. Working with young people, their families, and other agencies such as schools, youth organisations and more to provide non-clinical wellbeing support and signposting.
If you are worried about something, need to talk to someone, or would just like to learn more about what services or activities are available for young people – our Coordinator can help. Patients can be seen at Whitecliff Surgery or if you’re at school they can visit there too.
We can talk about any support you might need, and information can be shared about local activities and groups as well as local and national services that might be of interest to you. You can also talk via phone or text message if you would prefer.

As well as supporting the Practice’s Young Patients, our YP Coordinator works with patients who are Care Leavers. A ‘Care Leaver’ is defined as any individual who has spent time in care as a child and is now between the ages of 16 to 25 years old.
