The Blandford PCN Social Prescribers are key to the delivery of our Wellbeing Service. Research indicates that people’s health and wellbeing is often impacted upon by social, financial and environmental factors, so our Social Prescribers have built up excellent links with partner organisations so that they can signpost our patients to a wide range of help and support.

Our Social Prescribing Wellbeing Team has worked with local volunteers to establish our Health Champion Network. The Health Champion Network offers our patients the opportunity to take part in FREE volunteer led groups. Click here to find a local group – Health Champion Network
Social Prescribers in our wellbeing team can help with a wide variety of issues such as:
Carers Support – general advice and support, signposting and referral to external support, assistance with applications for attendance allowance and blue badge parking permits
Military Veterans Support – information and support for veterans, signposting to appropriate support networks, support with referrals to Veterans Trauma Network & Prosthetic Service
Dementia Support – help with memory assessment tests, referral to Memory Assessment Service, advice and support regarding memory loss and dementia, referrals for specialist help, help with applications for Attendance Allowance and other support
Young Patient Support – general advice and guidance, signposting to specialist services, and befriending.
Learning Disabilities Support – advice and support, signposting and referral to external help, support with referrals to other organisations, help with applications to external bodies.
